Compliance Monitoring Data Entry - Inspection Conclusion Data Sheet (ICDS)

There has been confusion about entering the very important ICDS data in ICIS.  There are business rules associated with the questions that do not allow for certain answer combinations.  Below is a clarification of the business rules and ICIS screen shots to help.

Instructions for Data Entry Into the ICDS Portion of ICIS (found on page 17 in the ICDS Manual)

 

This section of the Manual describes step-by-step instructions for entering information into the ICDS edit screen, as shown in Figure 9.  Once you are in the ICDS portion of ICIS, there are five questions that the inspector should answer based on the inspection that he/she conducted. 

 

Question #1: “Did you observe deficiencies (potential violations) during the on-site Inspection?”  Select Yes or No to the above question.

If Yes to question #1 above, select the appropriate deficiency from the “Deficiencies Observed” pull down list and go to question #2.  For a complete list of “Deficiencies Observed” in ICIS, please refer to page 19.

If No, then go to question #4: “Did you provide general Compliance Assistance in accordance with the policy on the Role of the EPA Inspector in Providing Compliance Assistance During Inspections?”

Question #2: “If you observed deficiencies, did you communicate the deficiencies to the facility during the inspection?”  Select Yes or No to the above question.

If Yes to question #2 above, then proceed  to question #3. 

If No, proceed directly to Question #4 “Did you provide general Compliance Assistance in accordance with the policy on the Role of the EPA Inspector in Providing Compliance Assistance During Inspections?”

Question #3: “Did you observe the facility take any actions during the inspection to address the deficiencies noted?”  Select Yes or No to the above question.

  • ·         If Yes to question #3 above, select the appropriate “Action taken” from the “If yes, what actions were taken?” pull down list.  For a complete list of “Corrective Actions” in ICIS, please refer to page 42
  • ·         If you have selected "Reduced Pollution” as the “Action taken", then click on “Pollutants” to select a pollutant from the available choices.
  • ·         Next, a pop-up window will open that provides the pollutant choices for either “Water Pollutants” or “Air Pollutants.”  Select the appropriate "Pollutant" choice and go to question #4. 

IMPORTANT NOTE:  If the answer to question #3 is No above, then proceed directly  to Question #4 “Did you provide general Compliance Assistance in accordance with the policy on the Role of the EPA Inspector in Providing Compliance Assistance During Inspections?”

Question #4:“Did you provide general Compliance Assistance in accordance with the policy on the role of the EPA Inspector in Providing Compliance Assistance During Inspections?”  Select Yes or No to question #4 above, then go to question #5.

 

Question #5: “Did you provide site-specific Compliance Assistance in accordance with the policy on the Role of the EPA Inspector in Providing Compliance Assistance During the Inspection?”  Select Yes or No to the above question.

 

The inspector can add comments about the inspection or evaluation in the optional "Summary of Comments.”
